- Lesion Pattern: Characterized by patchy inflammation with distinct skip lesions (e.g., inflammation in the stomach, followed by normal tissue, then inflammation in the colon).
- Inflammation Depth: Transmural (involves all 5 layers of the gut wall).
- Pathological Consequences:
- Acute inflammation is highly liable to perforation and fistula formation.
- Healing occurs via fibrous tissue, resulting in narrowing, stricture formation, and subsequent intestinal obstruction.
- Disease Pathways: Operates via three distinct pathways: inflammatory, obstructing, and fistulizing.
- Clinical Hallmarks:
- Presence of perianal lesions (fissures, piles, or fistulas).
- Extensive or repeated surgical history (e.g., an appendectomy) in a young patient.
- Gold Standard Diagnosis: Visualization of a non-caseating granuloma. These are located very deep in the tissue wall, meaning they are rarely captured on standard mucosal endoscopic biopsies and typically require a surgical biopsy to visualize.

2. Clinical Presentation & Abdominal Pain Localization

The primary complaints of IBD are abdominal pain and changes in bowel habits.
- Crohn's Pain Profile: Severe, colicky abdominal pain dominates the clinical picture. It is not related to meals, often causes absence from work, and requires emergency department visits for IV fluids.
- Misdiagnosis Risk: Crohn's pain is frequently confused with an acute abdomen, leading to unnecessary surgical interventions (e.g., appendectomy, cholecystectomy, or exploratory laparotomy).
- Bowel Habit Changes: Can present as chronic diarrhea, alternating diarrhea and constipation, or constipation alone.
- Abdominal Pain Localization:
- Periumbilical Pain: Originates from the small intestine.
- Epigastric Pain: Originates from the duodenum or stomach.
- Referred Back Pain: Indicates a duodenal or pancreatic origin.
- Pelvic Pain: Originates from the colon.

3. Mimics of Crohn's Disease (Small Intestinal Diseases)

Several conditions present identically to Crohn's Disease and must be systematically ruled out:

Malignancy Adenocarcinoma of the small intestine, sarcoma, or lymphoma. Presents with chronic diarrhea (with or without blood), colicky severe recurrent pain, and symptoms that wax and wane in young/middle-aged patients. Surgical and oncological evaluation.
Intestinal Tuberculosis (TB) Resurgent infection caused by the ingestion of unboiled, infected milk. Exact carbon copy of Crohn's (young age, severe pain, chronic bloody diarrhea, multiple surgeries) but presents with a normal pulmonary X-ray. Must perform QuantiFERON test first before endoscopy. Requires two biopsies: one in formalin (histology), one in saline (PCR for TB bacilli). Steroids are absolutely contraindicated (causes severe disease flare).
Behcet's Disease Rheumatological condition sharing the small intestine as a target organ. Must ask about oro-genital ulcers (concurrent genital and aphthous mouth ulcers). HLA-B51 positivity is helpful but not mandatory. Requires urgent Ophthalmology evaluation to screen for eye involvement.

4. Ulcerative Colitis (UC) Specifics & The "Rule of Three"

Unlike Crohn's, Ulcerative Colitis is strictly localized to the colon.
- Inflammation Depth: Limited to the mucosal and submucosal layers only (never transmural; no fistulas or perforations occur).
- Anatomical Classifications:
- Local Proctitis: Extends from the rectum to the sigmoid colon. Has the best prognosis and is managed locally via suppositories and enemas (Steroids/Pentasa).
- Left-Sided Colitis: Extends from the rectum to the splenic flexure.
- Pancolitis: Involves the entire colon up to the cecum. Has the worst prognosis and can cause backwash ileitis (inflammation spilling into the terminal ileum).
- Primary Symptom: Chronic bloody diarrhea appears first, followed by tenesmus, mucus, and diffuse colicky pelvic pain.

⚠️ The Rule of Three: Amebiasis vs. UC

Young females presenting with chronic bloody diarrhea, tenesmus, mucus, and pelvic pain are frequently misdiagnosed with amebiasis based on stool analyses.
* The Rule: Allow exactly two trials of anti-amebic treatment. If symptoms persist, the third step must be a Colonoscopy to confirm UC. Do not waste further time on anti-parasitic therapies.


5. Extra-intestinal Manifestations (EIMs) & Autoimmune Coexistence

Extra-intestinal manifestations affect 10% to 20% of IBD cases and can precede gastrointestinal symptoms by 1 to 2 years (indicating a worse prognosis).
- Eye Involvement: Scleritis and episcleritis. This mandates immediate intervention and directs therapy directly toward Biologic treatment.
- Skeletal Involvement: Arthritis and low back pain. Differentiate from independent Ankylosing Spondylitis using HLA-B27 testing and an MRI of the sacroiliac joint.
- Skin Involvement: Erythema nodosum.
- Hematological System: IBD induces a hypercoagulable state, making patients highly susceptible to thrombosis.
- Autoimmune Disease Clustering:
- Endocrine: Hypothyroidism (very common, often family-wide) and Type 1 Diabetes.
- GI Overlap: Celiac disease (considered "cousins" with Crohn's).
- Dermatological: Psoriasis.

🚭 Smoking and IBD

Smoking (nicotine) is absolutely forbidden in Crohn's Disease. It activates the disease and makes the patient highly liable to severe complications, specifically vascular issues, lower limb ischemia, and thrombosis.

Conventional Treatment

Biologic Therapy

Biologics are indicated when conventional therapy fails or severe EIMs are present.
- Remicade (Infliximab): A chimeric antibody; it is the best agent for closing fistulas. It carries a risk of anti-drug antibody formation over time.
- Stelara (Ustekinumab): Excellent biologic choice for managing strictures and fibrosis.
- Loss of Response Protocol: If a patient relapses, verify compliance and order a Trough level and Anti-drug antibody test:
- Scenario A (Antibodies Present): Change to a different biologic family (e.g., switch to Stelara).
- Scenario B (Low Trough / No Antibodies): Increase the biologic dose (increase frequency or add an ampoule).

Surgical Indications in Crohn's Disease

"Surgery and Crohn's are not friends." Surgery is never first-line because it frequently triggers fistulas and disease recurrence. Surgery is strictly indicated for:
1. Obstruction due to a fixed fibrotic stricture (inflammatory strictures resolve with medical therapy; endoscopic balloon dilation of fibrotic strictures leads to high recurrence rates).
2. Peri-appendicular abscess or active infection.
3. Fistula refractory to medical treatment.
4. Dysplasia (specifically high-grade dysplasia in UC, which requires a Total Colectomy).


8. Pregnancy, Fertility, and IBD

The peak age of IBD onset (around age 20) directly intersects with childbearing years.

Medication Safety in Pregnancy

Three Golden Rules for a Successful Pregnancy

  1. Timing: Conception must occur during clinical remission to ensure a normal course.
