1. History of Upper Gastrointestinal Bleeding (Hematemesis)

Phase 1: Analysis of the Attack (Assessing Severity)

The primary clinical goal is to determine if the bleeding episode is mild, moderate, or massive.
- Volume/Amount: A volume of $≥$ 1 Liter of blood indicates a Massive attack.
- Color: Indicates the retention time of blood in the stomach.
- Bright Red (Fresh): Indicates a Massive attack, short gastric transit time, and sudden stomach fullness.
- Coffee Ground (Altered): Indicates a Mild attack, long duration in the stomach, and conversion of hemoglobin by hydrochloric acid (HCl) into acid hematin.
- Clots: Large blood clots described by the patient as "lumps like liver" indicate a Massive attack.
- Hemodynamic Status: Indicators of instability include:
- Loss of consciousness, shocked state, or being unable to walk.

General Examination Checklist

  1. Level of Consciousness: Gastrointestinal bleeding can precipitate hepatic encephalopathy.
    - Mechanism: Hypoperfusion to a diseased liver (acute-on-chronic failure) combined with the digestion of blood proteins by intestinal flora, which rapidly increases ammonia production.
    - Management: Administer laxatives (to wash out blood) and prophylactic antibiotics.
  2. Body Weight / Build: Note if the patient is average, overweight, or wasted. Cachexia (wasting) suggests malignancy.
  3. Skin Color:
    - Jaundice: Suggests liver disease or biliary obstruction.
    - Pallor: Indicates the severity of anemia.
    - Cyanosis: Central cyanosis indicates shock (severe blood loss). Respiratory cyanosis suggests lung metastasis (from HCC), pleural effusion (due to hypoalbuminemia), or hepatopulmonary syndrome.
  4. Special Facial Characters: Check for acromegaly, Cushingoid features, thyrotoxicosis (exophthalmos), facial palsy, malar flush, or an infantile look.

    - Hernia: Check for a para-umbilical hernia and elicit a cough impulse.

  6. The 3 Ps:
    - Pulsation: Epigastric pulsations (right ventricular hypertrophy or aortic aneurysm).
